In consideration of the assignment to the CLIENT by Eastbridge Building Group the (“Assigned Employee”) has received, read, and understands the "Code of Safe Practices" for Eastbridge Building Group. The assigned employee has agreed to conform to all the practices, rules, and regulations relating to a safe work performance. The assigned employee understands that failure to follow the safety procedures will result in disciplinary action up to and including discharge.

All assigned employees have the responsibility to report all unsafe conditions or violations of the Code of Safe Practices to a supervisor or other management personnel in order to minimize the potential of injury to fellow workers. The assigned employee is encouraged to inform an immediate supervisor of any hazards at the worksite without fear of reprisal, and should the assistance create any such action or related intimidation, he or she is encouraged to contact his or her supervisor at Eastbridge Building Group or an on-site manager.
